Handicap Parking Permits
The Town Clerk's office will need the completed DMV Form (PDF) and an NYS Driver's license or Non-Drivers ID.
Please note: The DMV form explains how to apply for a parking permit or license plate for persons with severe disabilities.
A copy of the NYS License or Non-Driver's ID and the completed/signed form are acceptable to be brought in by someone other than the applicant.
- Who is eligible for vehicle plates for disabled persons or a parking permit for disabled persons?

The NY State Vehicle and Traffic Law defines the permanent disabilities that qualify. If you have a permanent disability that allows, you can get vehicle plates for reserved parking from the DMV or a parking permit from your local government. It is illegal to park in a parking space reserved for persons with disabilities if you do not have the correct vehicle plates or parking permits. You can use the vehicle plates or parking permit in New York State and most other states.
- How do I get a parking permit for persons with disabilities?

You and your healthcare provider must complete the Application for a Parking Permit or License Plates for Persons with Severe Disabilities (PDF) (MV-664.1) available below.
You also can have your healthcare provider (MD, DO, DPM, or NP) complete a statement on their letterhead. According to the law, the information must describe the disabling condition and verify that the disability qualifies. Bring the completed form or letter to Guilderland Town Hall, Town Clerk's Office.
There is no fee for a permit. Remember that the DMV does not issue parking permits.
Please note: If renewing a blue permanent disability tag that was initially issued by the Town of Guilderland, you only need to present a Driver's License AND the Expired/Expiring Tag. An applicant or their representative can fill in the top section of the Department of Motor Vehicles form while at Town Hall. You do not need to revisit your physician.
