Insurance Requirements
Building permit applications will not enter the review process until we receive up-to-date insurance certificates. Review time varies by season, please inquire with building department staff for the current timeline.
Required Forms for Contractors
- Liability - Accord Form
- Worker's Compensation - Form C-105.2, Accord Forms are not acceptable
- Disability - Form DB-120.1
- Or Worker's Compensation and/or Disability exemption form CE-200
Required Forms for Homeowners
- Certificate of Attestation of Exemption (CE-200)
- Obtain electronically at the Workers' Compensation Board website
- Print the Certificate and sign and date before submitting to the Building Department
All forms will have the Town of Guilderland as the Certificate holder.
